This letter is typically required to submit an insurance claim. We provide your physician with a simple, ready-to-sign template.

  • Contact us to request our referral overview and collaboration agreement. Once in place, all we need from your office is a signed Letter of Medical Necessity for each eligible patient. We handle the consultation, fitting, ongoing support, and billing from there.
